For years, conversations about healthy aging have focused on numbers: blood pressure, cholesterol, body weight, daily step counts and the number of birthdays someone has celebrated.
New research is drawing attention to another question that may be just as meaningful: What can your body actually do?
A large study published in JAMA Network Open suggests that practical measures of physical fitness, including balance, agility, lower-body strength and cardiovascular fitness, are strongly associated with longevity among older adults.
The findings add to a growing view of fitness as more than structured exercise. How effectively a person can stand, rise from a chair, move across a room and sustain physical effort may provide important information about functional health as the years pass.
Researchers Followed More Than 13,000 Older Adults
The study included 13,423 community-dwelling adults in Taiwan who were at least 65 years old. Their average age was approximately 73.
Participants completed standardized fitness assessments during 2015 and 2016. Researchers then connected those results with national health records and followed the group for a median of seven years. During that period, 1,631 participants died.
Rather than looking only at whether participants reported exercising, the research team measured what they were physically capable of doing.
Seven assessments covered four major areas of fitness:
- Cardiovascular fitness
- Muscular strength
- Flexibility
- Balance and agility
That distinction matters. Someone can describe themselves as active, but a physical performance test provides a more direct picture of strength, mobility, endurance and control.
Balance and Agility Produced Some of the Strongest Associations
One of the standout assessments was surprisingly simple.
Participants performed an 8-foot up-and-go test. Starting from a seated position, they stood up, walked approximately eight feet, moved around a marker, returned to the chair and sat back down. Faster completion reflected better performance.
Those in the highest-performing group had an adjusted mortality risk that was about 59 percent lower than the lowest-performing group during the follow-up period.
Another test required participants to stand on one leg with their eyes open for as long as possible, up to 30 seconds. Those in the highest performance group had an adjusted mortality risk approximately 50 percent lower than those in the lowest group.
These results do not mean that being able to balance on one leg will make someone live longer. The study was observational, so it cannot establish cause and effect.
Instead, abilities such as balance and quick, controlled movement may act as visible signs of something larger: the body’s remaining functional reserve.
Leg Strength Matters Too
Another important finding involved a movement most people perform every day without considering it a fitness test: getting out of a chair.
Researchers used a 30-second chair-stand assessment to evaluate lower-body strength. Participants repeatedly stood up and sat down during the allotted time.
People in the highest-performing group had an adjusted all-cause mortality risk about 45 percent lower than those in the lowest-performing group.
That makes intuitive sense from a functional perspective.
Strong legs do much more than help someone squat a heavier weight in the gym. Lower-body strength contributes to walking, climbing stairs, getting out of a vehicle, recovering balance and maintaining independence.
For younger fitness enthusiasts, leg training may be associated with performance or appearance. As people grow older, however, the ability to produce force through the hips and legs can become increasingly connected to everyday freedom.
Cardio Fitness Remains an Important Part of the Picture
Strength and balance did not make cardiovascular exercise irrelevant.
Participants also completed a two-minute step test designed to assess cardiorespiratory fitness. Those performing at the highest level had an adjusted mortality risk approximately 42 percent lower than the lowest-performing group.
Researchers observed a graded relationship between cardiovascular performance and mortality, reinforcing the importance of maintaining aerobic capacity as part of healthy aging.
The broader message is therefore not to choose between cardio, strength or balance.
It may be to train all three.
The Strongest Result Came From Being Well-Rounded
Perhaps the most interesting finding appeared when researchers stopped looking at individual exercises.
They created a composite fitness index combining performance across all seven assessments.
Participants in the highest overall fitness group had an adjusted mortality risk about 61 percent lower than participants in the lowest group.
That finding supports a practical idea that sometimes gets lost in modern fitness culture.
There may not be one magical exercise, one perfect workout format or one physical quality that determines whether someone is fit.
A person who can walk with endurance, stand with control, move quickly when needed and generate useful strength may have something more valuable than excellence in one isolated exercise: physical capability across multiple systems.
Fitness After 65 Is About More Than Looking Fit
The study also challenges the idea that fitness should primarily be judged visually.
Body composition has its place in health and performance, but appearance cannot fully reveal whether someone has the balance to recover from a stumble, the leg strength to rise from the floor or the cardiovascular capacity to remain active for an extended period.
Functional fitness asks a different set of questions.
Can you move confidently?
Can you control your body?
Can you carry yourself through daily life without every physical task feeling difficult?
Can you preserve enough strength and endurance to continue doing the things that matter to you?
Those questions become increasingly important with age.
The Biggest Opportunity May Be for People Starting From the Lowest Fitness Levels
One of the more encouraging observations from the research is that much of the difference in mortality appeared between the lowest-performing group and those performing better.
The researchers noted that this pattern suggests potentially important gains when people move out of the least-fit category.
That changes the conversation.
Healthy aging does not necessarily require turning every older adult into an athlete.
For someone with very low functional fitness, improving the ability to stand from a chair, walk confidently, maintain balance or tolerate several minutes of continuous activity may represent meaningful progress.
The goal does not always need to be extraordinary performance.
Sometimes the most valuable fitness achievement is becoming more capable than you were yesterday.
What This Could Mean for the Future of Fitness
The study arrives as the fitness industry continues expanding its focus beyond weight loss and appearance toward longevity, mobility and quality of life.
That evolution could have important implications for gyms and fitness professionals.
A program designed for long-term health may increasingly combine traditional resistance training and cardiovascular exercise with movements that preserve balance, coordination, mobility and real-world physical capability.
For an older adult, progress might therefore be measured not only by pounds lifted or miles completed, but also by the ease of standing, walking, turning, climbing, carrying and recovering balance.
These are ordinary movements.
Their importance is anything but ordinary.
A Fitness Test Is Not a Crystal Ball
There is an important caution.
Nobody should interpret these tests as a personal life-expectancy calculator.
The researchers specifically described the findings as associations. Fitness was measured at one point in time, smoking information was unavailable, and the participants able to complete the assessments may have been healthier than some members of the broader older population.
The study was also conducted in Taiwan, so researchers noted that results should be generalized to other populations carefully.
The authors also cautioned that balance and mobility assessments should involve appropriate safety screening and standardized procedures, particularly for people at risk of falling.
The significance of the research is therefore not that everyone should immediately time themselves standing on one leg.
It is that functional fitness may deserve considerably more attention as an indicator of how well the body is aging.
The Fitness Living Takeaway
The future of healthy aging may be less about chasing one perfect health number and more about protecting the abilities that allow us to participate fully in life.
Build cardiovascular endurance.
Maintain muscle.
Train the legs.
Challenge balance safely.
Keep moving.
And perhaps most importantly, do not assume that becoming less capable is simply an unavoidable consequence of getting older.
The new research suggests that what the body can do may tell an important story about how well it is aging.
For a fitness industry increasingly interested in longevity, that may be one of the most useful stories of all.
